If we do not accept your insurance, we offer a time of service discount.

How do I schedule my new patient appointment?
 

  1. Click on the button below to go to our patient portal.

  2. You will answer "NO" when asked if you have been to our clinic before.

  3. Fill out your patient information.

  4. Choose the doctor you wish to see.

  5. Select your appointment date and time.

  6. SELECT "CONTINUE" ON NEXT SCREEN!

  7. Complete the information requested on the following screens. This will create your file and provide the necessary information to move forward. After these pages are submitted, you will receive a link to provide additional details related to your condition and history.

  8. This link takes you to our encrypted paperwork site. Enter your email and you will receive a code to enter into the site. Complete your paperwork and hit submit.

IMPORTANT: Both the intake and additional paperwork must be submitted prior to your appointment to avoid the need to reschedule your appointment.

What to expect on your first visit...

The initial visit, or New Patient Exam, typically consists of the following:

  • Consultation

  • Initial Exam,

  • Neck and Low Back X-rays (if necessary)

  • Review of X-rays

  • Chiropractic Adjustment

  • Therapy (either Electrical Muscle Stimulation, Cold Laser, and/or Dry Needling).

 

This generally can be done in an hour, however, we do ask that you plan for an hour and a half to complete this visit depending on the detail of exams and review of your health history.
